57^th Annual Michigan Onsite Wastewater Conference

This is your official exhibitor registration form.  The 57^th Annual Michigan Onsite Wastewater Conference will again be held at the Kellogg Center in East Lansing, Michigan on Tuesday - Thursday, January 8-10, 2008.

Each exhibit will cost $300 per booth, and will include one full conference registration for your company representative.  You can register additional company representatives or booth personnel for the full conference at $80 each, or one-day registrations are available for $55 each.  If you have any questions, contact Dan Milan (989) 773-9938 or Mike Stephens (517) 339-8692.

In order to provide the best opportunities for our exhibitors and our attendees, we have enclosed an Exhibitor Agreement, which must be signed in order to reserve space at this year’s conference.  Exhibits are sold on a first-come first-served basis.

*You do not need to submit a separate conference registration for the contact person listed for registration above, but please include the additional registration fees for each additional person with your booth payment.  For any one-day attendees, please indicate the day they will be attending as well.

Exhibit registrations will only be held with full payment and signed Exhibitor Agreement.  Unpaid registrations do not reserve your booth space for any length of time.  Exhibit space will be reserved on a first come first served basis.  Confirmations will be sent within 2 weeks of receipt of your paid registration.  Please call Mike Stephens (517) 339-8692 if you do not receive a confirmation within that time.
